I/we further consent to the use of my child’s/children’s likeness and/or photo(s) by St. Victoria for use in instructional, informational or promotional materials/purposes. In the event of an emergency and I/we cannot be reached, I/we grant authorization for any necessary medical procedures that must be performed on my/our child.
