Note: all volunteers receive a 25% tuition reimbursement toward the end of the Faith Formation year for any children registered for Faith Formation at any grade level!

I the undersigned recognize that St. Victoria Catholic Church agrees to provide Faith Formation classes according to published schedules and policies. In consideration of St. Victoria Catholic Church providing said service, I hereby agree to abide by published terms and conditions. I/we further consent to the use of my likeness and/or photo(s) by St. Victoria for use in instructional, informational or promotional materials/purposes. In the event of an emergency and my emergency contact cannot be reached, I grant authorization for any necessary medical procedures that must be performed on me.

Safe Environment Archdiocese Mandates
Volunteers 18 years and older must complete all 3 items listed below before volunteering. A key to our commitment to promoting safe environments in our parishes, Catholic schools, and communities is for all persons in positions of trust in regard to minors and vulnerable adults to fulfill the Enhanced Essential 3 requirements (EE3). All volunteers over the age of 18 must complete a parish-paid background/drivers check, sign a code of conduct, and complete the Virtus safe environment training. Volunteers 17 years and younger need to submit two letters of reference and sign a Code of Conduct. Each need to be updated every three years. Student volunteers 17 years and younger will becontacted by Emily Klinker with details about the submitting your Code of Conduct and Reference Letters.

Failure to maintain compliance will result in being asked to step down from your volunteer position.

Maintain compliance in ALL Safe Environment mandates from the Archdiocese of St. Paul Minneapolis Attend Catechist/Leader training (annually), retreat (annually) meetings (weekly) Be willing to witness my faith with the youth of the parish according to the teachings of the Catholic Church.
